OK, I think this is some sort of PE. I will go out on a limb & say I think you have a pretty good PE except for one spot on the worm, which is a pretty large "jump". I say that because of the teardrop-shaped stars...like they are in the right place most of the time, then take a brief but significant excursion every so often.
have you trained PEC for this mount?
I would start by looking for a nick or burr on one of the drive gears. make sure everything in the driver train is properly adjusted. Then balance the scope carefully, and train PEC.
Maybe others here will have other suggestions, but that is where I would start.
